Agape Rural Cardiac Access Pilot

Agape Mobile Heart Foundation’s primary initiative is the Agape Rural Cardiac Access Pilot.

This program is designed to expand access to cardiovascular services in rural, tribal, and underserved communities by bringing coordinated mobile outreach, cardiovascular diagnostic access, care navigation, telehealth-supported follow-up, and community-based partnerships closer to the people who need them. Services may support children, adolescents, adults, and older adults when appropriate clinical oversight, provider orders, qualified personnel, and partner workflows are in place.

What The Program Provides

The Agape Rural Cardiac Access Pilot is designed to support:

• Mobile cardiovascular access events
• Primary vital signs collection before EKG or echocardiography encounters
• EKG and portable echocardiography access when clinically appropriate and ordered
• Licensed physician and cardiologist interpretation
• Pediatric specialist involvement when pediatric services are provided
• Telehealth-supported care coordination
• Follow-up care navigation and referral coordination
• Rural, tribal, hospital, clinic, and community partnership development
• Outcome tracking and reporting

Why This Matters

Many children, families, adults, and older adults in rural, tribal, and underserved communities face long travel distances, delayed evaluations, limited specialty access, transportation challenges, and financial barriers to cardiovascular care. These barriers can delay early detection, diagnosis, treatment, and follow-up care, increasing the risk that cardiovascular conditions go unrecognized or unmanaged.

Agape exists to help bridge that gap.

How The Pilot Works

Agape partners with rural hospitals, clinics, tribal health organizations, community organizations, and other qualified healthcare partners.
Partner sites help identify community needs, establish appropriate patient workflows, and coordinate scheduling and outreach.
Primary vital signs are collected before EKG or echocardiography encounters and made available for provider records and care coordination.
Patients receive access to cardiovascular diagnostic services when clinically appropriate, properly ordered, and supported by qualified personnel.
Studies are reviewed and interpreted by appropriately licensed physicians and cardiologists, with pediatric specialist involvement when pediatric services are provided.
Patients are connected with follow-up care, specialty services, telehealth-supported coordination, and care navigation when needed.
Agape tracks program activity, access measures, follow-up coordination, and outcomes to support accountability, grant reporting, program improvement, and responsible expansion.

Telehealth-Supported Coordination

Agape may use low-cost telehealth-support tools, including an iPad Pro and tripod setup, to help connect rural patients, local staff, and licensed providers during outreach events and follow-up coordination.

Clinical interpretation remains with licensed providers and partner healthcare professionals.
