Agape Rural Cardiac Access Pilot

Agape Mobile Heart Foundation’s primary initiative is the Agape Rural Cardiac Access Pilot.

This program is designed to expand access to cardiac diagnostic services in rural and underserved communities by bringing mobile cardiac ultrasound access closer to the people who need it most.

What The Program Provides

The Agape Rural Cardiac Access Pilot focuses on:

• Mobile cardiac screening events
• Portable echocardiography access
• Licensed physician/cardiologist interpretation
• Telehealth-supported coordination
• Follow-up care navigation
• Community partnership development
• Outcome tracking and reporting

Why This Matters

Many rural patients face long travel distances, delayed evaluations, and limited access to cardiac diagnostic services. These barriers can delay early detection and make it harder for patients to receive timely follow-up care.

Agape exists to help bridge that gap.

How The Pilot Works

Agape partners with a rural hospital, clinic, church, community organization, or local site
A mobile cardiac outreach day is scheduled
Patients receive access to cardiac ultrasound services closer to home
Studies are reviewed through licensed physician/cardiology partnerships
Patients are connected with follow-up care resources when needed
Agape tracks outcomes to support future rural healthcare funding and expansion

Telehealth-Supported Coordination

Agape may use low-cost telehealth-support tools, including an iPad Pro and tripod setup, to help connect rural patients, local staff, and licensed providers during outreach events and follow-up coordination.

Clinical interpretation remains with licensed providers and partner healthcare professionals.
